Toll-Free LOA (Letter of Agency)

Thank you for choosing Virtual PBX. In order to complete the porting of your toll-free number, you will need to fill out the complete the form found below.

After your LOA has been submitted, you will receive a PDF document from our Support Team. Please print and sign the document you receive, then, fax it back to Virtual PBX at 888-825-0800 OR 408-971-1050.

IMPORTANT: If you need to port more than 4 toll-free numbers, please fill out a separate form for the other toll free numbers.
